Original signed passport with at least six months of remaining validity and two blank visa pages, that is valid for at least 90 days after the visa expiry date.
- Visa Application Form
One completed and signed Visa Application Form. An application has to be filled out completely. If the answer is none, write “NONE”. If a question does not apply, type “N/A”.
- Passport-type photo
One color photo that meets the following strict criteria: taken within the last six months, against an all white background, printed on high-quality photo paper, 45 mm tall x 35 mm wide exactly, provide the full frontal view of your head with your face centered in the middle and displaying a neutral (non-smiling) expression, eyes open, ears visible, no background light or shadow over your face, neither eyeglasses nor headwear except for religious purposes, sized such that the face takes 70-80% of the photograph.
- Proof of legal stay or residence status (applicable to non-U.S. citizens)
You must provide the original and photocopy of your valid certificates or visa of stay, residence, employment or student status, or other valid certificates of legal staying provided by the relevant authorities of the country where you are currently staying.
- Tourist invitation
An original or a copy of a letter of invitation issued by a Belorussian hotel or a tourist agency which is registered as a tourist company by the Belorussian authorities.

- Medical Requirements
All visa applicants are required to provide a copy of their health insurance policy card, along with their policy documents that is valid in the Russian Federation for the visa period (can be provided by Travelex, Travel Guard, RoamRight, Allianz Global Assistance, Travel Mate, Seven coners, International Medical Group (IMG), etc.).
- Birth certificate. A copy of the applicant’s birth certificate.
- Parents’ passport copies. A copy of the information pages of both parents’ signed passports.
Applicants who have USSR or Belarusian roots must provide official documents proving that they do not possess Belarusian citizenship.

- Business Letter
An original or a copy of a letter of invitation from a company or organization in the Republic of Belarus, which is registered as a legal entity by the Belorussian authorities.
The letter of invitation should be type on company’s letterhead with registration number, date of issue, signature of an official and a corporate seal. It should also indicate the period of stay in the Belarus.
