Types of E-visa:

Clear scanned copy of information page of passport. Passport has to be signed, with at least six months of remaining validity and two blank visa pages.
2. Passport-style photo
One recent digital 2 x 2 passport photograph, in color, front view and with a plain/light background.
3. Business invitation (applying for Business visa only)
A business invitation letter from the sponsoring company.
4. Letter from the hospital (applying for Medical purpose visa only)
Letter has to be typed on a letterhead of the hospital where the treatment is suppose to happen.

The visa application for minor must be signed by both parents and notarized. A copy of a parent’s driver’s license or utility bill can be used for proof of residential address in the US.

Journalists affiliated with a media company going to India for journalism purposes or for tourism purposes are required to submit a letter from their company stating whether the applicant will be going for journalism or tourism only.
